- CMS Student/ Parent Athletic Handbook– effective for 24-25 school year. Please review. Required for sports participation.
- Physical Examination Form– every student is required to have one. You will not be able to practice/try-out without one. “Must be signed and dated May 1st of the current year through June 30th of the succeeding year.”
